After filling the whole square, the bolt was inserted.  With a few rubber washers against the side of the bed, a fender washer on the outside, the nut was slowly tightened down pulling the bolt through the mettle.  The hole time, watching to make sure the carriage bolt it's self was not spinning.

After removing the fender washer, the carriage bolt has a new, snug fit.

After cutting some rubber urethane and lightly gluing to the inside of the fender to act as a pad between the fender and the bed side, the fenders were bolted into place.
